patients with multimorbidity
I'm suggesting this 8 years on.
"Multimorbidity" is now commonly used for two or more chronic diseases in a patient. Note that the "chronic" part may make it inappropriate in an acute context, but otherwise it is probably a valid equivalent of "pluripatológico".
See these NICE guidelines published in Sept 2016, "Multimorbidity: clinical assessment and management"
