Spanish to English Medical Medical (general)
This is from a Venezuelan medical report. It appears at the top of the page as a header:

RIF: V-###
M.S.D.S.: 64###
C.M.C.: 8###
Where RIF is the tax ID code and CMC is the Colegio Medico de Caracas.


It could be Ministerio de la Salud y Desarrollo Social, but I don't know why that would have a number after it.
