عاد بشق الأنفس، عاد بجهد جهيد، عاد زحفاً

The expression "clawed his way back" is one unit. You can't translate one word without the others.

The expression means: returned with great effort against great odds, as if one inch at a time.

Both suggestions of Hassan and Mai above are great. They convey the effort involved in the return. I would like to suggest other ways (not necessarily better) to express the same thing:

عاد بجهد جهيد
عاد زحفاً

كان قد عاد من غيبته السـياسـية بجهد جهيد
