Hi all,
This phrase has appeared in the terms and conditions of a competition held in Mexico. It is a document that has to be submitted by participants in order to be eligible to receive a prize if they win. I'm not familiar with Mexican documentation and would appreciate any clarification as to what this form is and what it's rough translation would be in US English, if possible.

With so little to go on and the example of the form I have provided a link to in the Reference comments, it looks more like 'registration' of a third party than 'notification'. Only putting a low confidence level as we don't have enough information to be sure.

: as per your Colombian weblink. Third-party preferably hyphenated. Otherwise, too many permutations and combinations for 'radicación' in Robb and West, the latter including 'formal acceptance of jurisdiction over a case'.
