Hello. This is a document from Spain. It is called "Designación de cuenta bancaria y comunicación de datos bancarios".
The document also says:
"Datos del interesado o representante legal
Datos bancarios (domicilio entidad bancaria, denominación de cuenta (tipo de moneda))

A cumplimentar por la entidad de crédito
Certifico, a petición del interesado que figura anteriormente y a efectos de domiciliación del importe que vaya a ser transferido por la Secretaría General del Tesoro y Política Financiera, la existencia de la cuenta referenciada, cuyo titular es el mismo."

You could simply translate "designación de cuenta bancaria y comunicación de datos bancarios" as "notification of bank details", as the Spanish basically says the same thing twice.
