Protection of the repaired tissues is most essential during the first 6 weeks following surgery. The patient is educated regarding proper use of the brace and crutches. The brace will be locked in 0° of extension for crutch ambulation and while sleeping for up to 4 to 6 weeks. Weight bearing is initially limited to toe-touch weight bearing to partial weight bearing with crutches based on surgeon instruction. In the initial protection phase, the patient is also educated regarding frequent elevation of the extremity and application of ice for pain control and reduction of inflammation. Use of a combined cryotherapy and compression unit 3 to 5 times per day for 20 to 30 minutes at a time is advised for the first 4 to 6 weeks postoperatively. Any signs of atypical healing, including signs of infection, should be communicated to the surgeon immediately. Patellar mobilizations are performed immediately postsurgery to encourage mobility of the patella along the trochlear groove, prevent development of arthrofibrosis, and aide with the progression of knee ROM. Once the incisions are healed, scar-tissue massage is performed to minimize scar-tissue adhesions.
