With over 25,000 Syrian refugees recently arrived in Canada, a significant need for primary care healthcare services is expected in the coming weeks and months. Physicians and front-line healthcare workers across the country are beginning to assess and treat a range of physical, emotional and mental health needs for this wave of refugees. However, providing appropriate care to non-English-speaking refugees can involve significant challenges, particularly with limited translation support available.
To help address this, a free, secure online tool has been developed to allow patients to complete a full history and mental health screening in Arabic or Armenian, with the results automatically translated to English for the clinician to review and add to the patient record. Collecting this information in the patient’s first language can help ensure a more complete and accurate history is collected, while also increasing the likelihood of sharing sensitive issues. More.

These forms reflect a masterpiece and testament to the dedication, talent and service to their respective communities by all of those participants.
To my best knowledge (and I welcome and would appreciate any related information otherwise via < shfranke @ hotmail.com>), no hospital, HMO, network, health center or private specialty clinic in the US has developed and uses similar language-enabled forms in their EMR systems (that said, The University of Texas M.D. Anderson Cancer Center in Houston, Texas may be the only proximate exception, per information inside its multilingual website).
One would hope that those medical professionals and their gracious bilingual associates in Canada who developed these Arabic and Armenian versions might later produce a similar version of the registration form and the PHQ in Persian ("Farsi").

I will email you personally with more details, but I'm pleased to let you know that we can easily enhance any of our freely available forms to support Farsi, with just a little help with the translations. All we do is provide a spreadsheet (usually a shared online Google spreadsheet) for people to contribute the translated strings. Once the translations are done, we just upload the strings to the online form.
