Help me please with a standard text of notarial confirmation of translator's signature in Hebrew.

Doesn't the notary provide this?

Thank you for replies.
Notary does not provide it but only confirm translator's signature in both languages, and Hebrew is the one that he does not know.
I'll ask in Hebrew forum Thanks
