Es muy extraño decir "acta al margen". Acta es partida/certificado (certificate), pero según el contexto podría ser una simple nota/anotación.

minutes in the margin
"Acta" is usually "minutes" or "report", and the literal translation seems fine to me. No doubt certain colleagues will soon pooh-pooh my suggestion though....

Maybe it's just me, but I wouldn't normally use "certificate" as my first choice, unless the original were "certificado"....

: "Minutes" cannot be a translation of "acta" // "minutes" suggests the record of a meeting

Discussion
Pareciera ser simplemente: Nota al margen del acta = Nota al margen / Nota marginal / Anotación marginal.

Marginal notice allows the monitoring of the development of information concerning a person and his family and its modification in a civil status document.
It shall be positioned on the side, in the margin, of the civil status document.
It indicates, among other things, the nature, date and place of the event (birth, marriage, death, etc.) which is mentioned.
