Yes, I'm guessing the letter is from a medical professional, and these are their registration details, with 'registros' meaning registrations rather than records.

You could be right. I've found the Facebook page of the Colegio de Especialistas en Retina Médica y Quirúrgica de Jalisco A.C., and there's a paragraph that says: Detalles
